Real time sync
Is it possible start a sync as soon as a file will be modified in the origin folder?

Sorry, but real time synchronization is not supported in SyncBackFree.
However, SyncBackPro/SE can be configured to watch for changes, and when changes are detected the profile runs automatically (and copies new/changed files from Source to Destination). You can configure this under:
Modify > Expert > When > Changes > "Run this profile when any files or directories are changed on source/destination"
I would suggest you to read the help file for the important points to consider when using OnChanges setting. With When > Changes settings page opened, press F1 to open the contextual help.

I set Syncback SE in that way but it trim when a file is open and try to copy the file + its temporary file.
Is it better avoid this? If yes, in what way?

SyncBackSE (by default) copies open/locked files from an internal drive using Windows Volume Shadow Copy service (VSS). However, if you don’t want open/locked files to be copied then you can try enabling the option “Do not use the Volume Shadow Copy service (VSS) to copy open files" under:
Modify > Expert > Copy/Delete > Advanced settings page.

Profile seems not to work, so:
I have deleted previous profile
I set: New profile > backup >
- Orig: \\FS1.XXX.YYY.it\USR1$\5314410
- Dest: C:\Users\5314410\Dropbox\Lavoro\SyncBack_CartAz5314410

Result: when a file changes in the source, the profile doesn't trim.
